The Michigan High School Athletic Association Medical History Form must be completed (both sides) prior to the exam. (Blank forms are available by clicking the link below, or in the main office and athletic office.)

The MHSAA requires that each student athlete have a completed physical on file after April 15th of the current school year and prior to any athletic tryouts or competition.

This MHSAA Medical History form must be completed on BOTH sides (including signatures) and on file in the Athletic Office prior to any athletic tryout. A physical is required each year for all student athletes. Physicals must be completed after April 15th of the current school year.
