Athletic Announcements
 Print the MHSAA Medical History Form 2008-09 This MHSAA Medical History form must be completed on BOTH sides
(including signatures) and on file in the Athletic Office prior to any
athletic tryout. A physical is required each year for all student athletes.
Physicals must be completed after April 15th of the current school year.
